1. About Fortitude Care

Fortitude Care provides disability support services including accessible and non-accessible transport, community access support, daily living support, domestic assistance, capacity building, group activities and related support services. Some information we collect may include sensitive information, including disability, health, support needs and safety-related information.

This policy applies to Fortitude Care's website, enquiries, referral processes, service delivery, staff/contractor engagement, communication with support coordinators and authorised representatives, and related administration activities.

Where appropriate and with consent or lawful authority, we may also collect information from authorised representatives, family members, guardians, nominees, support coordinators, plan managers, health professionals, allied health providers, hospitals, community organisations, government agencies or other people involved in a participant's support network.

Fortitude Care collects and uses personal information for purposes connected with providing safe, appropriate and reliable support services. These purposes may include:

  • Responding to enquiries and referrals
  • Assessing whether Fortitude Care can safely provide the requested support
  • Preparing service agreements, support preferences, consent forms, risk assessments and onboarding documents
  • Planning and delivering transport, community access, domestic assistance, daily living, capacity building and group activity supports
  • Communicating with participants, families, guardians, support coordinators, plan managers and authorised professionals
  • Managing participant safety, incidents, complaints, feedback and continuous improvement
  • Billing, payment, service administration and business record keeping
  • Recruiting, onboarding, allocating, supervising and paying workers or contractors
  • Meeting legal, regulatory, insurance, safeguarding and duty of care obligations

Sensitive information may include disability, health, mobility, medication, behaviour, risk, cultural or other sensitive details. Where Fortitude Care needs to communicate with external professionals, such as a support coordinator, GP, allied health professional, counsellor, behaviour practitioner, plan manager or emergency contact, we will seek appropriate consent unless there is an urgent safety concern or another lawful basis for disclosure.

Fortitude Care will only use or disclose personal information for the purpose for which it was collected, a related purpose that would reasonably be expected, with consent, or where permitted or required by law. Depending on the circumstances, we may disclose relevant information to:

  • Authorised family members, guardians, nominees or representatives
  • Support coordinators, plan managers, allied health providers, medical providers or other approved professionals

  • Government, regulatory or law enforcement bodies where required or authorised by law

We do not sell participant or client personal information.

7. Website forms, email and online communication

If you submit information through our website, email, online form, social media message or other electronic communication, we may collect the information you provide and use it to respond to your request, assess service suitability, arrange support or manage administration.

Electronic communication may not always be fully secure. Please avoid sending unnecessary sensitive information through unsecured channels unless required for the service request. Fortitude Care will take reasonable steps to protect information once received.

8. Storage and security of information

Fortitude Care takes reasonable steps to protect personal information from misuse, interference, loss, unauthorised access, modification or disclosure. These steps may include secure digital storage, restricted access, password protection, worker confidentiality obligations, staff/contractor training, secure disposal, and limiting access to people who need the information for their role.

Where information is stored by third-party technology providers, Fortitude Care will take reasonable steps to use providers with appropriate security measures.

9. Overseas disclosure

Fortitude Care's services are based in Australia. We do not intentionally disclose participant personal information to overseas recipients for service delivery unless necessary, authorised or consented to. Some technology or cloud service providers may store or process data using systems located outside Australia. Where this occurs, Fortitude Care will take reasonable steps to protect personal information and use appropriate service providers.

10. Accessing or correcting your information

You may request access to personal information Fortitude Care holds about you. You may also request correction if you believe the information is inaccurate, incomplete, out of date, irrelevant or misleading.

To request access or correction, contact Fortitude Care using the details below. We may need to verify your identity before responding. In some circumstances, access may be limited or refused where permitted by law, for example where access would impact another person's privacy or safety.
